Converting Serious Incidents into Improvement Opportunity to Transform Complete Process Safety Culture.

Process safety incidents usually has serious business, reputational and strategical implications. This paper will describe how a serious process safety incident (huge process fire in 2012) has transformed complete business approach towards safety in general and process safety in particular.

Leadership teams converted such unpleasant incident as an opportunity to revisit and correct gaps at strategic & operational priorities and approach towards business. post this incident site has decided to adopt DuPont 22 element of process safety model as the way to operate plant.

This consistent effort and learning form the incident converted to opportunity for improvement and business given strength to whole business. This paper will further describe how this effort has transformed cultural as sustainable change at every level across organization and afterwards no such incident repeated at site since last 8.5 years. Further process safety incident & severity rate both improved noticeable.
