(41au) Process Safety Knowledge Management - Moving from Data to Knowledge

All things change over time and PSM is no different. We can no longer just depend upon having the right information at hand. We must also have a process to capture, organize, maintain and deliver the right information as needed, when needed and where needed in order to prevent process deviations from becoming incidents. This presentation will first identify what process safety knowledge is not, then move on to define process safety knowledge, and introduce the elements and processes needed to add knowledge management to process safety knowledge. Finally, the presentation will briefly review gaps in process safety knowledge management (PSK) in terms of a well-known and recorded incident and suggest the two most commonly observed failures in PSKM based upon review of a number of infamous process safety incidents.
