(41d) Lessons Learned: Thermal Runaway Reaction Prevention System

The presentation will describe lessons learned from 29 years of improvements on a safety instrumented system (SIS) to protect against thermal runaway reactions. The discussion begins with a brief description of 1994 Thermal Runaway at Shell Chemical that initiated design of the SIS. Then the overview of the original design concept will be described. Finally, several step change improvements over the years will be discussed moving the system from a safety integrity level (SIL) of SIL 2 to SIL 3 design.
