This work focuses on the development of an approach considering the application of the bow tie methodology for monitoring and reporting the integrity of safety barriers (or controls) used in the prevention of collapse of dams which storage water for refinery process purposes, as well as for the mitigation of the consequences in case of materialization of this scenario. After the elaboration of the bow tie diagram with the technical specialists directly involved with dams safety, the integrity of the controls/barriers is determined based on information collected from each dam in the operational level. Following this, those results are validated and periodically reported to managers responsible for each control/barrier in order to properly support risk management, permeating all the relevant technical and hierarchical levels (including the board of directors).
