化工厂中执行的工作许可制度是防止在非常规作业中事故的核心管理制度， 但由于涉及人数多，非常规作业，可能的复杂工艺等等因素。 涉及工作许可的事故依旧是化工厂中最常见，最严重的事故。

本文通过分享十三个与工作许可有关的事故，讨论工作许可制度可能出错的环节并分享用于防范这些出错的优秀制度安排，对制定更加适用全面的工作许可制度提出建设性的意见。

The permit to work program is the key defense for chemical
site abnormal operational incident. However it involves many related parties,
abnormal operation, needs to conduct the complex isolation process, etc, the
incident caused by permit to work procedure failure still happened frequently
in chemical site.

The article shares 13^th incidents which related
with Permit to Work procedure failure and discuss the most easy failure parts
of PTW procedure and share the best practices of preventing these failures.
