ABSTRACT
Today, most organizations utilize several methodologies or tools to solve problems, however the results often disappoint. In special a world-class manufacturing or reliability organization be found that doesn’t have a fairly good handle on its problems. Reliable data in the computerized maintenance management systems (CMMS) and distributed control systems (DCS) provide solid evidence to support problem analyses so causes of problems can be identified and eliminated. Most all have a formal root cause analysis (RCA) program in place and apply a structured method to events exceeding their threshold criteria. It is for those organizations that an opportunity exists to step away from the smaller, day-to-day incidents that normally garner the majority of the organization’s attention and elevate the scope and focus of the problem-solving efforts onto a higher value plateau -- seeking to identify and eliminate systemic causes.Even though solutions for systemic causes generally consume more time and money to implement, once we begin to expose systemic causes, their elimination will: Prevent recurrence of past problems, Prevent future problems. (Focusing on and eliminating systemic causes is one of the ways you can be proactive with your RCA efforts) and Yield massive savings and improvements (in many cases across multiple functions of the organization)
