In the aftermath of a major catastrophic accident, one often hears that we should strive to drive our risks to “zero” -- in reality that is not a desirable objective for an individual nor a corporation. Terms such as, hazards, frequencies, probabilities, safety, and risk will be discussed to provide a common language to understand risk and how it can be evaluated to help make informed decisions. Overviews of different types of risk assessment studies used in the chemical oil and gas industries using a variety of risk mitigation practices will be discussed and presented to illustrate the extraordinary levels of mitigation systems that have been employed to manage risk by industry.
