Considered the worst chemical disaster in history, the release of toxic gas from a Union Carbide chemical plant in Bhopal, India 36 years ago changed the way chemical process safety is practiced throughout the world. As this situation unfolded, numerous layers of protection failed. This presentation will introduce the concept of "Layer of Protection Analysis", (LOPA), and use it to identify the safety layers that worked, and those that did not, and ask whether there are any similarities at your facility.
