This presentation will explain the journey, from knowledge gap recognition, to book writing and then the videos. It will focus on the new ?Incidents that define Process Safety? which is in large part based on materials donated by BP.
In discussions with young graduates or even experienced technicians it has become clear that many had not heard of Flixborough, Feyzin or Piper Alpha, and if they had, few of them received enough information to be able to transfer lessons to their current activities. BP initiated a project in early 2004 to develop a process safety teaching aid in the form of a booklet containing analyses of non-process and process industries major accidents against their Integrity Management programme. BP has offered this booklet to CCPS for wider publication under the title mentioned above. To complement this document, video animations are being developed to help disseminate widely and keep alive the main lessons from major incidents in a simple, user friendly way. The presentation will include a short overview of the main chapters of the book with relevant incident titles; and one or two of the videos depending on time available.
