Abstract: -

The benefits of reporting and investigating Near misses have long been established in the safety management systems, as they result in prevention of actual incidents from taking place. At Reliance Jamnagar in line with this belief, reporting and investigation of Process safety Near miss is given top priority by the management.

The paper presents the systems and benefits of process safety near miss reporting and investigation at Reliance, Jamnagar. Few of the key elements of it are: i) IT tool for reporting of Near miss, ii) categorization of near miss based on the risk rating, iii) investigation based on risk rating using the same procedure which is being used for Process Safety Incidents, iv) review of investigations results by the top management, v) reporting of near miss one of the key parameters in team KRAs.

Also the paper discusses few near misses which have helped in identifying opportunities for strengthening our Process Safety management systems. The incidents range from challenges to Equipment integrity, errors in operating procedures, faults in instrumented systems etc.
