Abstract
The
Celanese incident lessons learned portal consist of four levels: company lessons
learned, technology specific lessons learned, site specific lessons learned and
external incidents of interest. This
paper will present how we capture knowledge from previous incidents to retell our
story at various levels in the organization. It is designed to engrain learnings from past incidents into the corporate process
safety culture to prevent repeats.
Company Lessons Learned
Company
lessons learned use CSB type videos to share the key causes of the major
incidents that have occurred in company with all Celanese employees. The system includes the initial delivery
method and definition of how the messages are reinforced over time. They describe why we have established a
process safety system.
Technology Specific Lessons Learned
Technology
specific lessons learned are gained either by experience, past incidents, or
standard design. These learning's are
targeted to hourly workers in each technology, and describe what could go wrong
in that technology and the safeguards in place to prevent.
Site Specific Lessons Learned
This
is a common area on the process safety portal that gives everyone access to one
page that contains the information of the most significant incident lessons
learned that have happened at each site. Site specific lessons learned are used
for toolbox meetings, operator training, new leader process safety
certification, PHA preparation, and other process safety activities.
External Incidents of Interest
This
is a place to share with all the employees the lessons learned from external
incidents of interest based on relation that they have with our companies
manufacturing operations.
