Learning From Incidents / Near Misses To Drive Improvement

One element of a PSM program is the Incident Investigation process. Although data collection is good, and incident analysis is better, what really drives improvement are the lessons we learn and the actions we implement as a result of these incidents. Hindsight is twenty-twenty, that is true, but the challenge is how does an organization learn from the past. This session invites original papers on learning from process safety incidents and near misses, including investigation program management, utilizing trend analysis tools, sorting through the findings and implementing change to drive improvement.
