Human and organizational factors (HOF) and improper maintenance have led to number of accidents in chemical process industry, this notion was also strengthen after concluding an analysis of past accidents in chemical process industry between 1983-2013. Therefore, there is a need to include the HOF and maintenance into overall risk calculations. Quantitative Risk Assessment (QRA) study is usually carried out during the design phase of a project, therefore QRA is selected to integrate the effect of HOF and maintenance.
3D Computational Fluid Dynamics (CFD) consequence analysis of potential loss of containments events is widely used in chemical process industry. In this study an analysis will be performed to quantify the HOF with the aid of 3D CFD models which can provide an enhanced detail about consequences of an undesired event.
By considering HOF and maintenance aspects, Location Specific Individual Risk (LSIR) and Individual Risk Per Annum (IRPA) can be calculated in more comprehensive way compare to conventional practices. A case study is also presented with the proposed method to calculate the risk.
