On June 13, 2013, the Williams Geismar Olefins Plant experienced a catastrophic accident: a reboiler for the propylene fractionator overpressured and ruptured, resulting in two employee fatalities. The U.S. Chemical Safety and Hazard Investigation Board (CSB) investigated the incident and published an investigation report, which issued recommendations to Williams and presented Key Lessons for the petrochemical industry. In this paper and presentation, the CSB presents its investigation findings. Topics discussed to prevent a similar incident include using the hierarchy of controls, establishing a strong process safety culture, and developing a robust process safety management program.
