A Six Sigma Kiazen event was held to identify areas of improvement in Process Hazard Analysis (PHA) One opportunity that was identified was to give feedback to new PHA facilitators on the quality of their PHAs. Internal compliance audits revealed that new PHA facilitators struggled to meet regulatory requirements. To address this issue, a Quality Check was implemented for all new PHA facilitators. The Quality Check is a tool that reviews all parts of the PHA and rates them on compliance, giving higher weight to those items that identify and prevent major incidents. The PHA facilitator is required to submit a completed PHA to the corporate PSM group. The corporate PSM group scores the PHA and provides individual feedback to the PHA facilitator on how to improve. These adjustments are made before they complete their next PHA and/or updates may be required to the Quality Check PHA.
