(53v) Comparison of Causal Factors for Dozens of Loss of Containment Incidents in R&d and Manufacturing Facilities--Many in Common

Causal factors for dozens of major loss of containment incidents that occurred worldwide at process industry production sites (many used as case study examples in CCPS books and process safety courses) have been compared with causal factors for over three dozen loss of containment incidents in industrial, government, and academic R&D facilities globally. This analysis was done as part of the work on a soon to be published CCPS guidelines book concerning prudent process safety practices for labs and pilot plants. A tabulation of fatal events in R&D facilities will be presented along with summaries of selected case study examples to appear in the book.
