A lot of attention is given to “near misses” or “near hits”. It is my view that these terms downplay the potential for severity of outcome. What if near misses were viewed as just being “lucky”? I think most managers would agree that luck runs out and that it sounds a bit more uncertain as to the outcome. Ken Tague has spent 37 years in operations and corporate safety and is currently the Process Safety Manager for Archer Daniels Midland Co. He shares two incidents with completely different outcomes that shaped his appreciation of and passion for process safety.
