(10a) Approach of Human Factors in Risk Analysis and Operational Procedures: Stress Level Analysis of Operators in Performing Tasks

Process risk analyzes generally address operational aspects in their entirety. Human factors are commonly found as root causes of major accidents, which are not mentioned in the process risk analyzes. Include analysis of human factors in the process risks analysis is necessary in an attempt to cover and exhaust the possibilities of accidental hypotheses in a facility with risk of major accidents. This work shows the implementation of the inclusion of human factor analysis in the process risk analysis, and therefore the inclusion of human factors in the operational procedures, as a point of improvement identified in the risk analysis. For this, the Operator Action Event Tree technique was used to determine the level of stress that operators are subjected to in performing procedures, which can lead to human errors and contribute to the occurrence of major accidents or not to block the effects of an initiating event.

As análises de riscos de processo geralmente abordam em sua totalidade aspectos operacionais. São comumentes encontrados como causas raízes de grandes acidentes fatores humanos, que não mencionados nas análises de riscos de processo. Incluir análise de fatores humanos nas análises de riscos de processo e se faz necessário na tentativa de abranger e esgotar as possibilidades de hipóteses acidentais em uma instalação com risco de grandes acidentes. Assim, este trabalho mostra a implementação de inclusão de análise de fatores humanos nas análises de riscos de processo, e, por conseguinte a inclusão de fatores humanos nos procedimentos operacionais, como ponto de melhoria identificado nas análises de riscos. Para isto, utilizou-se metodologia baseada em árvore de decisões para determinar o nível de estresse que operadores estão submetidos na realização de procedimentos, que podem levar a erros humanos e contribuir para a ocorrência de grandes acidentes ou não bloquear efeitos de um evento iniciador.
