This paper studies the practices & programs that support reporting of Near misses, minor unsafe conditions, incidents and quantitative risk assessment strategies in our organization for analyzing and learning from past events of operational failures, injuries, safety and environmental incidents.

A near miss is a leading indicator to an incident that, if scrutinized appropriately, can prevent injuries and accidents. Some persons may be tempted to write off near misses as “no harm, no damage” situations, but organizations who track near misses, determine how and why they occurred, and take corrective action can prevent similar or more serious incidents from occurring in the future. Fatima Fertilizers Limited believes that, “We don’t look at what happened; we look at what could have happened”.

Keeping in mind the importance of near miss reporting, FFL initiates drive and implement systems for

1. Capturing and increasing near miss reporting at whole site.

2. Logging data for analysis and using this valuable data for identifying gaps in existing process safety system.

3. Implementing sustainable improvement plans and safety campaigns (Safe hands, Safe Routines, Job Cycle Checks, Safe Drive etc) in our process safety systems for making our site a safer workplace.

These actions has improved the site performance which is reflected as Site has now over 45 Safe Million Man Hours, Total Record-able Injury Rate (TRIR) is below 0.2 since last 03 years and No Loss time injury since last 06 years.
