AFPM and API have been managing the Advancing Process Safety Programs since 2010. Through these programs, the associations have been able to collect a large amount of process safety data through API RP 754 collection, the event sharing database, and process safety site assessments. This paper will present the analysis of that data, the improvement opportunities identified , and the tools developed to assist sites in improving their process safety performance. One example of this is the identification of an industry opportunity to improve process safety events caused by open ended lines. Approximately 20% of process safety events are caused by the failure to isolate energy during equipment commissioning, draining, and unloading/loading activities. AFPM in conjunction with several member companies have developed a suite of practice sharing tools, called, âWalk the Lineâ to assist our members in developing a program to address these types of events.
