The mining industry defines the catastrophic event scenarios of its operations as Material Unwanted Events or MUE's. This definition aims to standardize the categorization of these scenarios and thus allow different activities in the mining area to establish performance and comparison criteria. Another important objective is to help prioritize these scenarios to be studied and appropriate risk management actions defined. This paper aims to present Vale's experience, so far, in operationalizing these risk analyses. It is also the objective of this work to present the lessons learned in using the Bow Tie technique when performing the hazard identification and risk analysis (HIRA) of the scenarios identified as MUE's. HIRA, Hazard Identification and Risk Analysis is one of the 22 elements of the Risk Based Process Safety (RBPS) system developed and developed by the AIChE/CCPS.
