We attend conferences to hear the experiences of others, learn lessons from past events and take that experience and learning back to our place of work. Yet a simple analysis of suggest there is a plateau in the progress to reducing the frequency of process safety incidents.
So perhaps the question to address is, âWhy are lessons not being learntâ?
This paper will highlight various data demonstrating the âplateauingâ of performance; highlight particular recurring indicators that need to focused on and look at how learning may be distributed to those that need to increase their knowledge of PSM. There will be a particular emphasis of the use of case studies in this approach.
The paper will particularly interest those still on the learning journey in PSM and those questioning alternative approaches.
