Repeat Incidents - Enough Already!

Human are one of key link to repeat incidents. Its observed that usually organizations focus on hardware improvement after an incident but comparative efforts to ensure that associated teams also consistently perform as required and remain alert with associated threats are only limited to training and awareness.
Neuroscience has proven that human brains thinks in pictures or key words i.e. Visual Thinking. This papers will describe how unique approach has been adopted by Fatima site to devised a comprehensive program to utilize this brain limitation in their support. This program not only helped in controlling repeat incidents (mainly related to injuries & Process Safety incidents) but also kept sense of vulnerability intact.
This program not only helped to improve safety performance of site. a site having around 900 contractors and around 800 employees able to avoid any lost time incident has been recorded since over 8 and half years and site is clocked over 57 Million Safe Man Hours.
