Mechanical integrity procedures and practices have taken center stage as various failure mechanisms have been found to be at the root of major loss of containment incidents. This session seeks papers that reflect on lessons learned with procedures, practices and tools that have stood up to the challenge and are integral to the maintenance, operations and risk management strategies of the facility. There has been an evolution from standards based compliance to continuous improvement to risk based programs. What’s next? What lies ahead? What are learning organizations doing on such topics as identification and inspection of corrosion mechanisms, use of operating integrity windows, reliability data management of instrumented safety functions, etc.?
